Group Health Insurance

Employee and dependents receive employer subsidized medical, dental, vision and pharmacy benefits available the first of the month following 90 days of continuous employment.

Employer Paid:

  • $25,000 life insurance with buy-up option
  • $25,000 accidental death and dismemberment
  • Short-term disability
  • Long-term disability

Employee Paid:

  • Supplemental life/accidental death and dismemberment
  • Dependent life/accidental death and dismemberment
  • Ambulance service

Paid Time Off:

  • Paid Time Off (PTO) is all purpose leave, and is accrued every two weeks.  Sellback options are available.

Accrual Rate:

  • 1 year – 3.69 hours
  • 2 years – 5.54 hours
  • 3 -4 years – 7.38 hours
  • 5-9 years – 9.23 hours
  • 10-14 years – 11.08 hours
  • 15 – 19 years – 12.93 hours
  • 20 – 24 years – 14.78 hours
  • 25+ years – 16.63 hours

401(k) Plan

Available the first of the month following six months of continuous employment.

Vesting Schedule:

  • Year 2 – 20%
  • Year 3 – 40%
  • Year 4 – 60%
  • Year 5 – 80%
  • Year 6 – 100%
